What You'll Do5
- 1Plan patient admissions in coordination with hospitals physicians community agencies referral sources
- 2Assess potential patients for hospice criteria
- 3Establish customer relationships collect pertinent data present agency services
- 4Admit patients develop initial care plan according physician present required paperwork
- 5Visit patients within service area provide nursing care instruction support to patients families
Requirements5
- 1Registered Nursing license South Carolina or eligibility for licensure
- 2Valid CPR certification
- 3Driver’s License reliable transportation
- 4Healthcare home care hospice experience preferred
- 51 year hospice experience preferred
