Overview
The LPN/LVN role at Providence at Home with Compassus involves holistic hospice nursing under RN supervision. This position focuses on delivering end-of-life care while supporting patients and families. It requires proactive anticipation of disease progression and skilled implementation of care plans.
What You'll Do11
- 1Provide hospice nursing services as directed by supervising RN
- 2Observe and report patient data for Plan of Care development
- 3Implement nursing interventions according to hospice policies
- 4Anticipate disease progression and manage symptoms
- 5Educate patients and families on care needs and changes
- 6Assist with insurance benefit understanding and cost-effective care management
- 7Document observations and communications accurately
- 8Manage caseload and communicate Plan of Care updates
- 9Collaborate with hospice team using critical thinking
- 10Perform triage and medication desk duties
- 11Follow Compassus/Providence policies and regulations
Requirements11
- 1California Licensed Vocational Nurse education
- 2Minimum 1 year LVN experience with 6 months home care preferred
- 3Infusion and Venipuncture Certification
- 4National Provider BLS certification within 30 days of hire
- 5Valid driver's license and auto insurance upon request
- 6Strong mathematical and language skills for documentation and calculations
- 7Knowledge of Medicare Conditions of Participation
- 8Ability to work in diverse healthcare settings with bodily fluid exposure
- 9Proficiency in Microsoft Office and EHR systems
- 10Commitment to professional boundaries and self-care strategies
- 11Experience with hospice and palliative care practices preferred
