What You'll Do11
- 1Provide personal care assistance including bathing dressing grooming and toileting
- 2Assist patients with safe transfers ambulation and mobility using proper body mechanics
- 3Support medication reminders as directed by the care plan
- 4Monitor and report changes in patient condition vital signs and daily living activities
- 5Perform light housekeeping tasks related to patient safety and care
- 6Document all care provided accurately in Homecare Homebase (HCHB)
- 7Communicate clearly with patients families and the care team to ensure continuity of care
- 8Follow infection control practices and maintain patient dignity and privacy
- 9Assist with range of motion exercises and other restorative care activities
- 10Coordinate with supervising RN/LVN for care plan updates
- 11Offer companionship and emotional support to patients
Requirements11
- 1Current Texas HHA certification
- 21+ years of experience providing personal care in home health settings
- 3CPR/BLS certification
- 4Texas driver's license and reliable transportation
- 5Ability to lift transfer and provide hands-on care safely
- 6Strong communication skills and professional demeanor
- 7Comfortable working independently in patient homes
- 8High school diploma or GED
- 9Familiarity with Homecare Homebase (HCHB) or similar EMR
- 10Bilingual English Spanish
- 11Experience with medically complex or high-acuity patients
