What You'll Do11
- 1Build and maintain patient panels using Spark and Airflow
- 2Design and execute evidence‑based treatment plans aligned with value‑based care goals
- 3Lead telehealth and in‑person visits delivering holistic care from 20 to 60 minute appointments
- 4Drive preventive health initiatives and chronic disease management programs
- 5Collaborate with multidisciplinary teams to ensure consistent documentation and workflow continuity
- 6Travel between locations to provide coverage as scheduled
- 7Utilize UpToDate and RubiconMD resources to enhance diagnostic accuracy
- 8Own scheduling flexibility by covering absentee teammates and opening shifts
- 9Maintain compliance with DEA and CPR/BLS certifications throughout employment
- 10Support operational excellence by standardizing clinical processes across sites
- 11Contribute to a culture that prioritizes patient outcomes over volume
Requirements9
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons of independent practice experience
- 3Active license and current ANCC or AANP board certification
- 4Federal DEA number preferred for full prescribing authority
- 5CPR/BLS certification required at start date
- 6Independent family practice experience including wellness, chronic care, and urgent visits
- 7Minimum 1 year of nurse practitioner licensure
- 8Proven ability to work autonomously in fast‑paced settings
- 9Strong communication skills for collaborative team interactions
