Overview
TFHC seeks a Family Medicine Physician to lead comprehensive outpatient primary care within a Patient-Centered Medical Home model. The physician will drive continuity of care and coordinate interdisciplinary teams across diverse communities in Hillsborough County. This role emphasizes preventive health chronic disease management and long-term patient outcomes.
What You'll Do12
- 1Provide comprehensive outpatient primary care including preventive services acute treatment and chronic disease management
- 2Perform patient histories physical examinations and medical diagnoses
- 3Order review and interpret laboratory imaging and diagnostic testing
- 4Develop treatment plans prescribe medications and coordinate follow-up care
- 5Make appropriate referrals for specialty care diagnostic services and additional treatment
- 6Maintain accurate electronic health record documentation
- 7Complete billing charges and clinical tasks according to organizational requirements
- 8Participate in daily care team huddles peer review and quality improvement initiatives
- 9Support efforts to close care gaps and improve clinical outcomes
- 10Provide clinical guidance and oversight to advanced practice providers nursing staff and other care team members
- 11Teach and supervise residents medical students and allied health professionals when applicable
- 12Support accreditation readiness clinical quality measures and organizational performance goals
Requirements10
- 1Graduate of an accredited medical school
- 2Completion of required postgraduate clinical training accepted by the applicable state licensing authority
- 3Board Certified or Board Eligible in Family Medicine or Internal Medicine
- 4Active and unrestricted Florida medical license
- 5Active DEA registration
- 6Current Basic Life Support certification
- 7Strong clinical judgment communication and leadership skills
- 8Commitment to continuing medical education and professional development
- 9Ability to work effectively with patients from diverse social cultural and economic backgrounds
- 10Proficiency with electronic health record systems
