What You'll Do10

  • 1Build patient care plans based on assessment data to improve outcomes
  • 2Lead coordination of care with families and healthcare providers to ensure safe, humane, and patient-centered environments
  • 3Design and implement discharge plans to sustain continuity of care post-ER visit
  • 4Drive efficient delegation and allocation of duties among nursing staff to optimize workflow
  • 5Ship and evaluate progress toward outcomes within 90 days, adjusting plans as needed
  • 6Collaborate with multidisciplinary teams to promote learning for patients, families, and team members
  • 7Coordinate health information delivery to patients and treatment teams to support informed decisions
  • 8Advance patient safety initiatives and contribute to continuous quality improvement
  • 9Contribute to a healing environment by modeling professional accountability and clinical excellence
  • 10Provide education and guidance to patients and families about care plans and resources

Requirements6

  • 1RN licensure in California is Required
  • 2BSN preferred
  • 3CPR or BLS certification from approved vendor per AH policy is Required
  • 4HLFS (Hospital Fire and Life Safety) is Required
  • 5ACLS certification from approved vendor per AH policy is Required
  • 6Acute care facility experience is Preferred
