You will own the Social Work Department at a FutureCare skilled nursing facility in Baltimore, directing medically related social services for residents across the continuum of care. You will lead a team, collaborate with interdisciplinary staff, and ensure compliance with state and federal regulations. What You'll Do8
- 1Direct the Social Work Department, supervising staff and programs to meet resident psychosocial needs.
- 2Lead daily interdisciplinary team meetings, ensuring resident care plans incorporate social work perspectives.
- 3Provide crisis intervention and counseling to residents and families facing illness, disability, and transitions.
- 4Address advance directives and identify appropriate medical decision-makers for resident care.
- 5Develop and implement individualized care plans that promote resident rights and dignity.
- 6Coordinate discharge planning across settings, ensuring smooth transitions and community connections.
- 7Deliver group and family services focused on psychosocial well-being, including grief support.

Requirements8
- 1Bachelor's degree in Social Work (required); Master's degree highly preferred.
- 2Current Maryland Social Work license in good standing.
- 3Prior experience in skilled nursing and long-term care environments.
- 4Management experience in an interdisciplinary setting preferred.
- 5Ability to lead and develop department staff.
- 6Strong interpersonal skills to relate to residents, families, and community agency personnel.
- 7Proven ability to prioritize tasks, meet deadlines, and handle distractions.
