Overview

You will deliver one-on-one support to adults and children with intellectual and developmental disabilities in home and community settings, helping them achieve goals from their Individual Support Plan. You will work with a multi-disciplinary team to promote independence and quality of life. What You'll Do8

  • 1Implement strategies from the Individual Support Plan to help individuals reach their outcomes.
  • 2Document services using EVV and daily service notes, following state regulations and Aveanna Healthcare policies.
  • 3Assist individuals and their team in developing and maintaining the Individual Support Plan.
  • 4Report changes or problems in the individual's condition to your manager immediately.
  • 5Guide and role model independence, helping individuals build life skills like cooking, budgeting, and personal hygiene.
  • 6Support individuals' preferences and interests, actively seeking community engagement opportunities.
  • 7Ensure the health and safety of each individual during all activities.
  • 8Maintain confidentiality and professional boundaries at all times.

Requirements10

  • 1Desire to assist individuals to live life to its fullest.
  • 2High school diploma or equivalency.
  • 3Active driver's license, current auto insurance, registration, and inspection if transporting.
  • 4Willingness to accompany individuals into the community and provide support.
  • 5Must be 18 or older.
  • 6Ability to work flexible hours, including overtime as needed.
  • 7Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
  • 8FBI clearance for non-PA residents.
  • 9Ability to remain calm and professional in stressful situations.
  • 10Effective problem-solving and conflict resolution skills.
