Overview
Own comprehensive care coordination across hospital units to optimize clinical outcomes and discharge planning. Drive collaboration with physicians, payers, and the care team to resolve barriers and empower patients and families. Work across multiple areas with diverse patient populations including high acuity pediatrics and complex chronic cases. This role blends clinical leadership with program knowledge to ensure safe, efficient transitions of care.
What You'll Do5
- 1Build and lead the interdisciplinary discharge plan with physicians, payers, and care team to achieve timely, safe discharge
- 2Communicate the discharge plan and updates to patients, families, and the medical team
- 3Identify and resolve barriers hindering effective patient care and utilization requirements
- 4Educate and mentor internal staff on case management concepts and best practices
- 5May supervise other staff and drive process improvements for patient populations
Requirements5
- 1Must have RN license in state worked
- 2Bachelor's degree in case management or health care for Case Manager path
- 3Master's degree in Social Work for Social Worker path and LMSW or LCSW or MSW with license within 6 months
- 4BLS certification for acute care assignments
- 52 years clinical experience with critical thinking and time management skills
