Requirements5
- 1Active license as an RN, Social Worker, Occupational Therapist, Physical Therapist, Speech Therapist, or in Rehabilitation Counseling.
- 2CCM or ACM certification required, or must be obtained within two years.
- 31+ year of case management experience.
- 4Full-time availability for Mon-Fri day shift with rotating weekends.
- 5High school diploma or equivalent (preferred).
