Overview
As a Case Manager at Valley Behavioral Health, you own client recovery and independence, guiding adults through personalized care plans with Case Manager certification. What You'll Do6
- 1Deliver psychosocial rehabilitative services as described by the Medicaid manual, adapting to each client's goals
- 2Build and maintain individualized care plans, adjusting interventions based on client response
- 3Document all services provided within Valley's standards, ensuring regulatory compliance
- 4Hit productivity targets and complete case notes within 24 hours
- 5Distribute medications accurately and monitor client adherence
- 6Transport clients to appointments, coordinating schedules and routes
Requirements5
- 1Case Manager certification required within 120 days of start date
- 2CPR certification obtained post-hire
- 3Driver's license and minimum age 21 for driving roles
- 4Previous social services or behavioral health experience preferred
- 5Bachelor's degree in Social Services, Psychology, or related field preferred
