Overview
ChenMed seeks a bilingual Care Facilitator in Tampa to lead patient interactions and support clinic operations. The ideal candidate will champion patient advocacy while driving operational efficiency across a rapidly expanding practice.
What You'll Do20
- 1Interact appropriately with patients and record information while greeting checking in checking out scheduling appointments
- 2Conduct lobby checks to maintain organization and cleanliness
- 3Serve as gatekeeper for PCP and specialist schedules ensuring top 40 patients are scheduled every 13 days
- 4Utilize computer equipment to add patient information print forms process appointments respond to emails and generate new patient packets
- 5Ensure accurate documentation of patient and emergency contact information
- 6Conduct love calls and send birthday greetings to commemorate special events
- 7Answer telephones take detailed messages and direct them appropriately
- 8Clearly communicate ChenMed clinical personnel information to patients and others
- 9Verify patient insurance via phone or website and collect necessary copays
- 10Assist with scheduling follow-up and specialist referral appointments
- 11Escort patients to exam rooms ensuring comfort and well-being prior to departure
- 12Act as HEDIS champion for PCP by providing visibility of HEDIS needs
- 13Report hospitalizations including HITS census ENS Ping
- 14Present patients with customer service surveys during checkout and escalate for service recovery
- 15Participate in daily and weekly huddles to provide patient updates
- 16Regulate transportation logistics including driver lists medication deliveries grocery deliveries driver behavioral challenges and ride coordination
- 17Maintain patient confidentiality and medical records
- 18Review patient information for completeness and accuracy
- 19Transmit correspondence by mail email or fax
- 20Perform other duties as assigned
Requirements10
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons experience in healthcare administration
- 35+ years working knowledge of medical insurance and EMR systems
- 4Bilingual Spanish proficiency required
- 5High school diploma or equivalent education required
- 6BLS certification for healthcare providers preferred
- 7Pay range $14.3 - $20.42 hourly with potential bonuses
- 8Flexibility to work evenings weekends and holidays up to 10 percent of time
- 9Spoken and written fluency in English
- 10Knowledge of ChenMed products services standards policies and procedures
