What You'll Do11
- 1Deliver compassionate patient-centered care managing acute and chronic pain in skilled nursing facilities
- 2Collaborate with primary care physicians and interdisciplinary teams to develop comprehensive pain management plans
- 3Conduct thorough patient assessments to identify pain sources and tailor individualized treatment strategies
- 4Utilize evidence-based practices to prescribe medications responsibly ensuring safe and effective pain relief
- 5Educate patients and families on pain management techniques fostering understanding and adherence
- 6Monitor and evaluate patient progress adjusting care plans to optimize outcomes
- 7Implement risk assessment tools and utilize prescription drug monitoring databases to mitigate opioid-related issues
- 8Coordinate care with nurses therapists and palliative care teams for patient-centered approaches
- 9Maintain accurate medical records using electronic health record systems
- 10Travel between care facilities in Minneapolis St Paul and surrounding suburbs
- 11Provide onsite pain management services eliminating transportation needs for patients and families
Requirements9
- 1Master's degree or higher in nursing or physician assistant education
- 2Accredited Nurse Practitioner or Physician Assistant program completion
- 31-2 years as a Nurse Practitioner or Physician Assistant
- 4Valid driver's license and auto insurance
- 5Reliable transportation to travel between care facilities
- 6Ability to build trust-based relationships with patients families and healthcare teams
- 7Experience with electronic health record systems
- 8Strong communication skills for interdisciplinary collaboration
- 9Passion for improving patient quality of life through pain management
