What You'll Do6
- 1Perform skilled physical therapy evaluations and develop individualized plans of care for home health patients
- 2Deliver therapeutic interventions focusing on gait balance strength and activities of daily living
- 3Monitor patient progress update treatment plans and document outcomes in the electronic medical record
- 4Conduct Start of Care visits and periodic revisits adjusting plans to meet patient goals
- 5Educate patients families and caregivers on safety transfers and home exercise programs
- 6Collaborate with interdisciplinary team to coordinate care and ensure smooth transitions and discharge planning
Requirements5
- 1Active Physical Therapist license in Pennsylvania or eligibility for PA licensure
- 2Recent home health experience
- 3EPIC EMR proficiency and OASIS documentation experience
- 4Ability to perform Start of Care and revisits in home health setting
- 5Willingness to travel within assignment area to provide home visits
