What You'll Do11

  • 1Conduct outreach in Alameda County neighborhoods schools and community organizations to promote mobile health clinics and center services
  • 2Develop culturally appropriate health education on preventive care immunizations and chronic disease prevention
  • 3Assist individuals with appointment scheduling and referrals to medical dental and behavioral health resources
  • 4Provide trauma informed responses and social service referrals to reduce barriers to care
  • 5Support mobile clinic operations by assisting with patient flow registration and community engagement
  • 6Document outreach activities referrals and follow up within 48 hours while maintaining HIPAA compliance
  • 7Lead projects that track key metrics and collaborate on initiatives supporting agency objectives
  • 8Demonstrate ethical integrity drive purpose and adapt to change while fostering cultural humility and collaboration
  • 9Maintain confidentiality and participate in required training and team meetings
  • 10Travel to community events and attend training relevant to key responsibilities
  • 11Monitor and report on performance metrics for assigned function areas

Requirements11

  • 1High school diploma or GED required with minimum two years experience serving underserved communities
  • 2Community Health Worker certification preferred bilingual English Spanish proficiency
  • 3Valid CA driver's license and auto insurance
  • 4Ability to lift fifty pounds frequently and drive a motor vehicle
  • 5Strong communication interpersonal skills and capacity to travel
  • 6Experience in non profit community settings and demonstrating customer service orientation
  • 7Proficiency with MS Office and Google Workspace products
  • 8Attend work regularly and adhere to policies on absences and timekeeping
  • 9Demonstrate cultural awareness and provide client centered care
  • 10Understand legal requirements confidentiality and risk management
  • 11Possess ability to process information and apply knowledge of community resources
