RN Case Manager Truhealth
Overview
Lead daily management and support of case management strategies for Medicare Advantage members in Pittsburgh. Provide in-person and telephonic patient visits. Coordinate with physicians and healthcare teams to develop and evaluate care plans. Requires self‑starter and team player who can manage multiple priorities with minimal supervision.
What You'll Do11
- 1Conduct health risk assessments for assigned members
- 2Initiate and revise care plans based on member needs
- 3Maintain active case loads and track patient progress
- 4Collaborate with interdisciplinary teams to plan and execute care
- 5Provide patient and family education using available teaching resources
- 6Coordinate outpatient discharge planning aligned with clinical circumstances
- 7Participate in managed care audits and generate reporting documents
- 8Develop monitoring tools and analyze data for improvement projects
- 9Attend conferences and participate in staff development programs
- 10Apply medical management criteria to resolve daily issues
- 11Research clinical questions and educate members on health access options
Requirements8
- 1Minimum 2 years clinical nursing experience with geriatric population
- 2Minimum 2 years managed care experience
- 3Experience in case management within a managed care setting preferred
- 4Current RN license and CCM certification preferred
- 5Valid driver’s license and motor vehicle insurance
- 6Graduate degree from an accredited RN program
- 7Bachelor’s degree preferred
- 8Current CPR certification for healthcare professionals
