Overview
Own the nutritional assessment and care planning for residents in a skilled nursing facility with weekend coverage. Lead evaluation of residents' dietary needs, implement care plans, monitor outcomes, and adjust as needed across the facility. Collaborate with team to review menus and resolve dietary issues, contributing to improved resident health. What You'll Do10
- 1Build comprehensive nutritional assessments for residents and develop individualized care plans
- 2Design and revise resident menus in collaboration with the culinary team and dietetic leadership
- 3Monitor resident outcomes, adjust nutrition interventions, and document care plan progress
- 4Collaborate with nursing and medicine teams to address complex nutrition-related conditions
- 5Educate staff and residents on dietary plans and nutritional benefits
- 6Participate in facility-wide quality initiatives to improve nutrition care
- 7Ensure compliance with New York State licensure and professional standards
- 8Utilize on-site resources to optimize patient care and support notetaking and reporting
- 9Contribute to continuous improvement efforts in clinical nutrition services
- 10Support weekend operations ensuring consistent, high-quality care
Requirements3
- 1Registered Dietitian license in New York State
- 2Degree in Food and Nutrition
- 3Experience with clinical nutrition care in hospital or nursing home, or similar clinical environment
