You will own patient care for home health visits in the Mesquite and Forney areas, delivering orthopedic physical therapy to adults in their homes. What You'll Do8
- 1Evaluate new patients within 48 hours of referral, applying orthopedic assessment techniques to determine functional limitations and therapy needs.
- 2Build and adjust treatment plans based on physician orders, reassessing progress every 30 days for PTA-supervised patients.
- 3Document each patient visit in Kinnser within 24 hours, ensuring compliance with Medicare guidelines.
- 4Submit progress summaries to physicians every 60 days and at discharge, using designated forms.
- 5Conduct monthly face-to-face care conferences with each PTA to review patient response and modify treatment plans.
- 6Teach patients and families home exercise programs, adapting techniques to the home environment.
- 7Coordinate with the care team during case conferences to ensure integrated care across disciplines.
- 8Participate in monthly inservice training to stay current on home health best practices.
Requirements8
- 11+ years of home health experience, with Kinnser proficiency preferred.
- 2Current Texas PT license in good standing.
- 3Graduate from an accredited physical therapy program (APTA accredited).
- 41+ years of orthopedic physical therapy experience, including joint replacements and post-surgical rehab.
- 5CPR certification (BLS for Healthcare Providers).
- 6Valid driver's license and reliable transportation for home visits.
- 72 reference checks required, including one from a direct supervisor.
- 8Experience with OASIS documentation is a plus.
