What You'll Do11
- 1Perform initial evaluation and functional assessment of newly admitted patients and patients returning from outside hospitalization
- 2Develop individualized treatment plan based on clinical findings
- 3Provide skilled physical therapy interventions for patients
- 4Monitor progress and modify interventions as clinically indicated
- 5Document in EHR timely and accurately all evaluations treatment progress notes and discharge summaries
- 6Educate patients and direct care staff regarding diagnosis treatment plans fall and injury prevention strategies
- 7Collaborate with physicians and other team members to ensure coordinated care for patients
- 8Serve in Safe Patient Handling and Falls committees as needed
- 9Schedule and anticipate length of assignment 20 hours per week Monday to Friday 8am to 4:30pm hours are flexible
- 10Conduct state and national criminal background checks
- 11Maintain current influenza hepatitis B and covid-19 vaccinations
Requirements6
- 15+ years building ETL pipelines with Spark and Airflow
- 2Doctor of Physical Therapy (DPT) from a program registered by New York State Education Department or accredited by CAPTE
- 3Pass the National Therapy exam NPTE and be licensed to practice Physical Therapy in NYS
- 4Minimum one year experience
- 5Submit resume with application
- 6Complete OMH required training including TRUST and annual standard trainings
