Occupational Therapist Job in Walnut Creek, CA | Rehab Without Walls
Overview
Occupational Therapist will own the full episode of care from evaluation to discharge, serving 5-7 patients per week in home and community settings. This role brings outpatient expertise to real life, delivering 1:1 therapy where patients practice skills in real environments. Join a collaborative interdisciplinary team of OTs, PTs, and SLPs with flexible scheduling and no weekend shifts. What You'll Do6
- 1Build treatment plans after comprehensive evaluations, documenting progress and adapting as needed.
- 2Re-assess treatment effectiveness at regular intervals, attending staffing meetings and family conferences.
- 3Conduct discharge evaluations and provide safety recommendations for home and community.
- 4Communicate patient needs to the treatment team, physicians, and families, ensuring coordinated care.
- 5Collaborate with other disciplines to enhance patient outcomes through integrated therapy.
- 6Drive patient progress toward independence in activities like managing prescriptions, driving, and returning to work.
Requirements8
- 1Bachelor's degree in Occupational Therapy from an accredited program
- 2Current, unrestricted OT license in California
- 3CPR Certification current
- 41+ years of work experience as an Occupational Therapist
- 5Knowledge of rehabilitation techniques for complex neurological diagnoses preferred
- 6Effective verbal and written communication skills
- 7Ability to lift 50 pounds and transfer patients up to 100 pounds
- 8Physical stamina for walking, standing, bending, and kneeling during sessions
