Overview
Own in home and senior living OT care at scale within the Geriatric House Calls model. You will assess, plan, and deliver one-on-one therapy to older adults in private residences and communities in Sacramento, CA. Join a clinician-operated practice that supports autonomy, flexible scheduling, and telehealth when appropriate. You’ll collaborate with an interdisciplinary team and contribute to meaningful outcomes that promote independence and aging in place.
What You'll Do6
- 1Build comprehensive evaluation plans and individualized care plans for older adults in homes and senior living settings
- 2Deliver evidence-based OT treatment sessions to improve mobility, independence, and functional outcomes
- 3Document accurately in FOX EMR following regulatory standards
- 4Collaborate with PTs, OTs, SLPs, and Fitness Specialists to coordinate care
- 5Educate patients, caregivers, and staff to promote safety and aging in place
- 6Provide telehealth / virtual care to extend access and continuity of care
Requirements7
- 1Current Occupational Therapy license in CA or eligibility
- 2Degree from an accredited OT program
- 3Basic computer and EMR proficiency
- 4CPR certification
- 5Ability to perform in-home and community-based care including lifting and transfers
- 6Access to transportation to patient homes and senior living communities
- 7Willingness to work across in-home and senior living settings

$80 - $110k per year
