What You'll Do8
- 1Conduct comprehensive assessments for acute kidney injury and chronic kidney disease patients
- 2Evaluate diagnose and manage renal conditions
- 3Develop personalized treatment strategies
- 4Collaborate with physicians nursing staff and interdisciplinary teams
- 5Educate patients families and staff on cardiovascular health and medication management
- 6Monitor patient progress and modify care plans accordingly
- 7Maintain precise EMR documentation
- 8Implement initiatives to reduce hospitalizations and return-to-hospital events
Requirements7
- 1Minimum six months experience in post-acute care environments
- 2Cardiology specialization preferred with training provided
- 3Active state license and NCCPA certification for APRN or PA
- 4Master’s degree in nursing or physician assistant program
- 5CPR BCLS/BLS certification
- 6Strong communication organizational and teamwork abilities
- 7Ability to work independently in clinical settings
