What You'll Do9
- 1Provide direct patient care following the personalized care plan and physician orders, documenting vital signs and responses to treatments in the EHR.
- 2Monitor patient behaviors including vital signs, symptoms, and treatment responses, reporting changes to the supervising RN.
- 3Collaborate with RNs to adjust care plans based on patient progress and rehabilitation goals.
- 4Build therapeutic relationships with patients by assessing physical, mental, and emotional needs to support recovery.
- 5Celebrate patient achievements and milestones during therapy sessions, reinforcing motivation.
- 6Administer medications and treatments as ordered, ensuring accuracy and safety within LPN/LVN scope.
- 7Educate patients and families on care plans, medication routines, and post-discharge instructions.
- 8Document care delivered in the EHR system, maintaining compliance with state regulations.
- 9Assist with patient mobility and daily activities, coordinating with physical and occupational therapists.
Requirements8
- 1Current LPN/LVN licensure as required by Texas state regulations.
- 2CPR certification.
- 3One year of experience in an inpatient medical-surgical or general hospital setting preferred.
- 4Rehabilitation experience preferred.
- 5Ability to work onsite in Houston, Texas.
- 6Strong communication and interpersonal skills for collaboration with RNs and patients.
- 7Compassionate, patient-centered approach to care.
- 8Flexibility to work shifts as needed, including weekends and holidays.
