Field Home Health RN PER DIEM Las Vegas NV
Overview
Registered Nurse administers skilled nursing care on an intermittent basis in patient residences following physician orders under supervision of Director of Clinical Services. Provide services according to plan of care and agency policies.
What You'll Do19
- 1Provides services in accordance with the plan of care
- 2Makes the initial evaluation visit and regularly reevaluates patient's nursing needs
- 3Initiates the plan of care and necessary revisions
- 4Provides services requiring substantial specialized nursing skills
- 5Initiates appropriate preventive and rehabilitative nursing procedures
- 6Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy
- 7Coordinates services
- 8Informs personnel of changes in patient condition and needs
- 9Counsels patient and family/significant others in meeting nursing and related needs
- 10Participates in and presents in-service programs
- 11Determines the amount and type of nursing needed by each individual patient
- 12Refers to Physical Therapist Speech Language Pathologist Occupational Therapist and Medical Social Worker those patients requiring their specialized skills
- 13Supervises and teaches other nursing personnel
- 14Conducts patient care conferences on assigned patients
- 15Participates in peer review and Quality Assessment and Performance Improvement as assigned
- 16Gives total patient care as needed
- 17Takes on-call duty nights weekends and holidays as assigned
- 18Completes and submits OASIS assessments reassessments transfers resumptions of care discharges and significant change in condition in accordance with Agency defined time frames
- 19Appropriately utilizes ICD-10 codes
Requirements5
- 1Graduate of an approved school of professional nursing and currently licensed in Nevada
- 2Two years nursing experience in home health care
- 3Acceptance of philosophy and goals of this Agency
- 4Ability to exercise initiative and independent judgment
- 5Familiarity with Wellsky software preferred but not required
Salary Insight
$104 - $125k per year
