What You'll Do9
- 1Design and implement therapeutic diets using evidence-based practices to address fluid sodium potassium and CKD-Mineral and Bone Disorder management
- 2Conduct patient assessments and develop individualized care plans in collaboration with physicians and other healthcare professionals
- 3Review laboratory results and educate patients on nutritional interventions tailored to their specific needs
- 4Participate in monthly interdisciplinary patient care conferences to coordinate treatment strategies
- 5Maintain quality assurance programs and contribute to continuous improvement initiatives
- 6Collaborate with medical directors nurses and allied staff to enhance patient care across diverse settings
- 7Drive process improvements to streamline clinical workflows and enhance patient outcomes
- 8Utilize advanced analytics to track patient progress and adjust treatment plans accordingly
- 9Mentor junior staff and share best practices within the renal care team
Requirements9
- 15+ years experience building ETL pipelines with Spark and Airflow
- 23-5 seasons managing complex patient populations in high-stakes environments
- 3AWS Certified Cloud Practitioner certification preferred
- 4CPC Card credential required
- 5BCBA certification valued for behavioral health integration
- 65+ years developing scalable nutrition solutions for large-scale operations
- 73-5 years implementing data-driven decision-making frameworks
- 8Proven ability to lead cross-functional teams in delivering exceptional patient experiences
- 9Expertise in regulatory compliance and documentation standards for healthcare settings
