Clinical Treatment Case Manager (Greenbrook)
Overview
Patient Access – Care Coordination specialist leading treatment journeys across multiple locations. Responsible for managing scheduling, insurance approvals, and patient communication to ensure timely care delivery.
What You'll Do9
- 1Guide patients from consultation through treatment completion ensuring a smooth supportive experience
- 2Coordinate appointments provider schedules and treatment timelines to maximize patient access
- 3Monitor patient progress and address barriers that could delay care
- 4Collaborate with Authorization Specialists providers and revenue cycle partners to maintain treatment plans
- 5Support prior authorization workflows and verify appropriate coverage throughout treatment
- 6Handle specialty pharmacy activities medication access and treatment readiness requirements
- 7Maintain accurate records financial estimates and documentation in CRM and EHR systems
- 8Respond to patient inquiries with empathy professionalism and urgency
- 9Drive high-quality experiences while meeting operational and compliance standards
Requirements5
- 1Bachelor's degree and minimum one year of patient care coordination experience
- 2Familiarity with AdvancedMD Salesforce EHR and Microsoft Office applications
- 3Knowledge of insurance benefits prior authorizations and healthcare scheduling processes
- 4Experience with TMS or Spravato therapy programs preferred
- 5Proficiency in patient care coordination and case management
