Overview
A full-time CNA or 75 Hour CHHA, joining a home health leader with 50 years of service, delivering in-home care to patients in Hillsborough County. You will work with a supportive team providing personal care, daily living activities, and compassionate, patient-centered support.
What You'll Do8
- 1Build and follow individualized work plans to provide safe in-home care
- 2Lead private in-home visits and Medicare bath visits across multiple locations
- 3Assist patients with activities of daily living and mobility
- 4Orient new patients and families in the Downtown Office for onboarding
- 5Coordinate with licensed professionals to ensure high quality home health care
- 6Document care provided and communicate patient status to the clinical team
- 7Maintain driver safety and reliable transportation for patient visits
- 8Support flexible scheduling and patient-centered care delivery
Requirements8
- 1Active Florida License/Certification as a CNA or 75 Hour CHHA
- 21 year of experience
- 3Strong communication and organizational skills
- 4Ability to work independently
- 5Valid driver’s license and reliable vehicle with valid insurance
- 6Commitment to compassionate, patient-centered care
