What You'll Do11
- 1Perform outbound calls to patients to understand clinical needs and connect them with resources
- 2Perform outbound calls to providers to schedule appointments and follow up on care
- 3Answer inbound calls from patients providers and other resources
- 4Serve as warm responsive point of contact for patients families and set clear expectations
- 5Follow up with patients to ensure needs are met and schedule future check-ins
- 6Monitor patient hospitalizations and follow up with care team members and outside resources
- 7Provide education materials and send communications to specialists for enrollments and appointments
- 8Help identify and prioritize patients for medical services each day
- 9Coordinate care transitions including post-hospital follow-up medication reconciliation and discharge instruction education
- 10Partner closely with on-site Advanced Practice Provider facility nursing therapy and social work teams
- 11Document all care management activities accurately and timely
Requirements5
- 1Licensed Practical Nurse LPN or Medical Assistant MA required
- 2Minimum 1-2 years experience in care coordination chronic care management primary care ambulatory care skilled nursing or hospital settings
- 3Knowledge of chronic conditions evidence-based guidelines prevention and patient education
- 4Experience with care management documentation and EHR systems preferred
- 5Associates degree or higher in clinical or health-related field preferred
