What You'll Do10
- 1Coordinate care by accessing resources and managing referrals and appointments
- 2Conduct outbound and inbound calls to patients providers and Village resources
- 3Complete non-clinical screenings and identify eligible programs under manager guidance
- 4Assist clinical teams with patient transitions through continuum of care tasks
- 5Identify member resources and collaborate with nurses dialysis clinics and health plans
- 6Communicate professionally with patients maintaining confidentiality
- 7Document patient interactions accurately and maintain HIPAA compliance
- 8Demonstrate exceptional two-way communication skills
- 9Align daily actions with DaVita mission to deliver compassionate care

Requirements11
- 1High school diploma or GED required
- 22+ years professional office experience
- 3Ability to handle confidential information discreetly
- 4Strong verbal communication skills
- 5Proficiency in Microsoft Office Excel and Outlook
- 6Prior experience with Electronic Medical Records preferred
- 7Commitment to patient satisfaction and service excellence
- 81-3 years clinical medical office experience
- 9CNA or MA experience is a plus
- 10Familiarity with insurance payer navigation
- 11Experience in direct patient care or support roles
